For federal income tax purposes, the contribution that is deductible is limited to the excess of money contributed over the fair market value of goods or services provided. We estimate the fair market value of one individual reservation to be $49.

Please fill out the registration form, print it out, and mail it to the address below.
*Registration must be received by September 16, 2011. The purchaser (you) must fill out personal information for yourself (and your guests if buying a table) so banquet coordinators can create name badges and have the correct food dish for participants.
