Youth Ministry Release Forms 2013 - 2014
These forms will take effect June 1, 2013 and remain in effect until May 31, 2014.

Parish Release Form Data Tracking
On and Off site Youth Ministry Guidelines
Form A - Annual Youth Ministry Parental Liability Waiver, Permission and Medical Information
Per each child attending any parish/diocesan youth event annually beginning anytime after May 31st.
Form B - Parental Consent for Youth to Participate and Consent for Emergency Medical Treatment
Must be filled out per each parish/diocesan event that the teenager is attending.
Form D - Adult Liability Waiver, Medical and Promotional Release
For all adults participating in parish/diocesan youth ministry.
Form OSA - Out of State Youth Ministry Parental Liability Waiver, Permission and Medical Information
For out of state or country trips, and pilgrimages.

These forms will take effect June 1, 2013 and remain in effect until May 31, 2014.
Form C1 - Young Adult Release and Waiver of Liability
For all young adults participating in parish/diocesan youth ministry.
Form C2 - Campus Ministry Release and Waiver of Liability
For all young adults participating in campus ministry activities.
